Support at Home – flexible care that moves with you

Support at Home is the new name for the Home Care Package program, funded by the Australian Government for eligible older Australians. It allows people to stay living safely in their own home with the right support.

Our Care Partners coordinate an extensive list of services, all aimed at helping you live well and stay connected. You are always at the centre of your care plan, and we move with you as your needs change. This may include help with personal care, home maintenance, meal preparation, transport, dementia support, respite for carers and many other forms of assistance that support independence at home.

Our team can also support you through the process of accessing aged care, explain the steps, provide information about costs, and help you understand what you are eligible for. We work respectfully with you and anyone you nominate — whether that is a family member, carer or friend — so everyone feels included and informed.

In home and community support

Daily life is easier when the right support is in place. Our in-home and community services help older adults live safely and comfortably at home. These include personal care, domestic assistance, individual support, respite, home maintenance and modifications recommended by an occupational therapist.
